Monthly fee:

$30 under 300 transactions per month
$90 300-999 transactions per month
$250 1,000-2,999 transactions per month
$750 3,000-14,999 transactions per month
$1,150 15,000-83,332 transactions per month
$1,400 over 83,333 transactions per month

Filing fee:

$0 1 state
$10 2-5 states
$25 6-10 states
$50 11-20 states
$75 21-30 states
$100 over 30 states
